How individual community support is planned

  1. The participant identifies activities, routines or skills that matter to them.
  2. We clarify the assistance needed before, during and after the activity.
  3. Transport, travel, activity expenses, risks and worker capability are agreed clearly.
  4. Feedback after support helps adjust future activities and maintain participant choice.

Questions answered before you decide

Can support include swimming, netball, parks or cafés?

It may, where the activity is chosen by the participant, appropriate to their goals, safely planned and consistent with funding and the service agreement.

Can a worker help with groceries or appointments?

Yes, these can be discussed as part of agreed community or daily-living support.

Is transport automatically included?

No. Vehicle suitability, provider travel, participant transport, kilometres and costs must be discussed and agreed.

Will the same worker attend regularly?

Continuity is considered during matching. No provider can promise one worker will never be unavailable, so leave and replacement preferences are discussed.
