When support coordination may be useful
You may be unsure what a plan includes, have services that are not working together, experience gaps or repeated changes, or need help comparing providers. With the participant’s consent, support coordination can help clarify priorities, identify options, establish services and build confidence managing supports.
Participants retain choice and control. We explain conflicts of interest, do not guarantee funding or provider availability, and record agreed actions and professional boundaries.

Support coordination FAQs
What does an NDIS support coordinator do?
A support coordinator can help a participant understand and use their plan, connect with providers, coordinate supports, build capacity and prepare for plan discussions within the scope of the funded support.
Does a support coordinator make decisions for the participant?
No. The participant remains central to decisions. Coordinators provide information and assistance while respecting choice, control, consent and lawful nominee or guardian arrangements.
Can you guarantee a provider or service outcome?
No. Provider suitability, availability and outcomes cannot be guaranteed. Support coordination helps explore options and manage connections based on the participant’s goals and circumstances.
Do you provide support coordination near Cockburn?
We accept enquiries across Cockburn and metropolitan Perth, subject to capacity and the participant having suitable support coordination funding.
Is support coordination the same as an LAC?
No. Local Area Coordinators and funded support coordinators have different roles and arrangements. Participants can contact the NDIS for guidance about who supports their plan.
Not sure whether this support is the right fit?
Arrange a free, no-obligation conversation about the participant’s goals, location, preferred schedule and support needs. We will explain the service and check current capacity and suitability. This is not an NDIS eligibility assessment, funding decision or guarantee of service.
Make your NDIS plan easier to understand and use
NDIS plans can involve several providers, budgets and decisions. Our support coordination service helps you understand the supports in your plan, identify appropriate providers and organise services around your goals. We explain options in plain language and help you prepare questions so you can make informed choices.
Support coordination may include
- Reviewing goals, funded supports and plan priorities with you.
- Researching suitable mainstream, community and NDIS providers.
- Helping establish service agreements and coordinated schedules.
- Supporting communication between providers when you authorise it.
- Identifying service gaps, risks and practical barriers.
- Building your confidence to direct and coordinate supports.
- Organising relevant information for plan review conversations.
Choice, transparency and independence
We explain potential conflicts and do not pressure you to use a particular provider. Our role is to strengthen your choice and control, not take decisions away from you. We cannot guarantee NDIA funding outcomes, but we can help you present clear, accurate information about your goals and support experience.
Support that respects the whole person
Access4All welcomes participants of all ages, cultures, faiths, identities and backgrounds. We support Aboriginal and Torres Strait Islander participants and acknowledge the importance of culture, family, Country and community. We also welcome people from migrant and refugee communities, including Iranian and Afghan participants. Tell us about language, cultural, gender, family or communication preferences that will help support feel safe and respectful.
Before support begins
We listen to what matters to you, confirm the service is appropriate, discuss availability and agree on responsibilities, boundaries, communication and costs. You remain involved in decisions and can provide feedback at any time.
Perth metropolitan availability
We accept enquiries from across the Perth metropolitan area. Our strongest local client communities include Cockburn, Spearwood, Kelmscott, Bertram and Baldivis, with additional support across surrounding Perth suburbs subject to capacity.
Frequently asked questions
What does an NDIS support coordinator do?
A support coordinator helps you understand your plan, connect with providers, coordinate supports, address barriers and build confidence managing services.
Can support coordination help before a plan reassessment?
A coordinator can help organise information about your goals, current supports and service outcomes. They cannot guarantee an NDIS funding decision.
Will I stay involved in provider decisions?
Yes. Recommendations should reflect your goals and preferences, and you retain choice and control over providers and agreed supports.
Looking for an NDIS support coordinator near you?
A nearby support coordinator can be useful when local provider knowledge and face-to-face meetings matter, but location alone does not prove the coordinator is the right fit. Ask about current caseload, response times, conflicts of interest, experience with the participant’s goals and how the coordinator will build the participant’s own capacity to direct supports.

Access4All checks support-coordination capacity individually across Perth. An initial conversation is free and does not guarantee that the service is funded, suitable or immediately available.
