A coordinator can explain funded-support categories, discuss participant goals and help identify practical priorities. They cannot guarantee funding decisions.
Connecting with providers
Support coordinators may research options, help compare providers and assist with establishing services. Participants retain choice and control over provider decisions.
Coordinating multiple supports
When authorised, a coordinator can help improve communication between services, identify gaps and address barriers affecting plan implementation.
Building participant capacity
The long-term aim is not simply arranging appointments. Support coordination should help participants strengthen knowledge, decision-making and confidence directing supports.
