NDIS Provider Capacity and Referral Checklist

This checklist helps support coordinators, LACs, plan managers, allied-health professionals and authorised family representatives obtain a useful capacity response without sending unnecessary personal information too early.

Minimum information for an initial capacity check

Information that can usually wait

Full medical reports, an entire NDIS plan, identity documents and unrelated personal history are generally not needed to answer a first capacity question. If Access4All appears suitable, we will explain what is required for safe onboarding and why.

A clear referral summary

“Adult participant in Cockburn seeks two recurring community-access shifts each week, preferably weekday afternoons. Support includes shopping, appointments and swimming. Calm communication, predictable planning and experience with autism are important. Plan managed. Participant has consented to provider enquiries.”

This illustrative summary lets a provider assess the likely roster and skill match. It does not guarantee suitability or acceptance.

What you can expect from our response

We will seek to distinguish between: likely capacity, more information required, a possible alternative schedule, and not currently suitable. We do not want referrers to mistake broad geographic coverage for confirmed capacity.

Frequently asked questions

Can I refer before a participant has chosen Access4All?

You can make an authorised capacity enquiry, but service should proceed only with the participant’s choice and the required consent or decision-making arrangements.

Should I send the whole plan?

Not for the first capacity check. Share only what is necessary and authorised. Further documents may be requested during onboarding where relevant.

Can I request a response by email?

Yes. Include your preferred contact method and enough detail for us to identify what needs confirmation.
