Access4All helps NDIS participants and authorised referrers explore community participation, support workers, in-home support, regular NDIS cleaning, support coordination and psychosocial recovery coaching across metropolitan Perth.

Capacity is confirmed individually. Send the participant’s suburb, requested support, preferred days and times, plan-management type and desired start date for a clear service-fit discussion.

For support coordinators and professional referrers

Support coordinators, local area coordinators, allied health professionals, hospital teams, plan managers, advocates and authorised family representatives may contact us when helping a participant compare providers. The participant retains choice and control, and consent or authority is required before personal information is shared.

Information that helps us respond efficiently

Do not send full medical records through the website form. If more information is required, our team will discuss an appropriate and secure way to share it.

  3. Regular NDIS cleaning for eligible household assistance.
  4. Support coordination to understand and implement funded supports.
  5. Psychosocial recovery coaching for eligible participants with psychosocial disability.

  3. If appropriate, we arrange an introductory discussion with the participant or representative.
  4. Services begin only after scope, pricing, schedules, responsibilities and the service agreement are understood and accepted.

Service exclusions and escalation boundaries

A referral is not an emergency pathway and does not guarantee acceptance, funding or an immediate start. Access4All does not use website referrals for crisis response, diagnosis, clinical treatment, legal advice or tasks outside the agreed service scope. If anyone is in immediate danger, call Triple Zero on 000. Urgent health or crisis concerns should be directed to the appropriate emergency, health or crisis service.

Operational information referrers may request

Before entering a service agreement, coordinators and participants may ask our team for the current documents relevant to the proposed service, including applicable worker screening and onboarding information, training and supervision arrangements, incident and safeguarding processes, insurance information, cancellation terms and feedback or complaints pathways. Requirements vary by service, so our team will confirm what applies to the referral.

Frequently asked questions

Can a support coordinator refer an NDIS participant?

Yes. A support coordinator or another authorised professional can submit an initial referral when the participant has chosen Access4All and consented to the necessary information being shared.

How can I check current NDIS provider capacity in Perth?

Send the participant suburb, requested support, preferred schedule and desired start date. Access4All will review current service capacity and explain whether the referral appears suitable.

What information should a professional referral include?

Include only information needed for initial screening: contact and consent details, suburb, goals, support requested, schedule, plan management, accessibility or communication needs and relevant service-delivery considerations.

Does submitting a referral guarantee service?

No. Service begins only after capacity, suitability, funding, scope, worker matching and onboarding requirements have been discussed and agreed.

Will the participant remain involved in decisions?

Yes. The participant retains choice and control. Access4All involves the participant or authorised representative in matching, scope, scheduling and service-agreement decisions.

Checking provider capacity for a participant?

Support coordinators, Local Area Coordinators, plan managers, allied health professionals, participants, nominees and guardians can contact us to check current capacity and participant-provider fit. A capacity enquiry is not an acceptance decision, and the participant or nominee remains central to provider choice.

  • Tell us the service, suburb, preferred schedule and participant goals.
  • Include communication preferences, worker preferences and relevant support considerations.
  • We will discuss suitability, consent, capacity and next steps with the nominated person.
